Lip Balm Holders

I chose to make these cute fall lip balm holders for a swap group I'm participating in....once I figured out the cutting/size (lol) these were very easy to make. My biggest hurdle was what/how to decorate the front?! Enjoy~
I've had a few people ask me for the poem used inside the holder....enjoy~
